By registering and submitting the form on this website, you are confirming that you are of legal age and affirming your acceptance of the terms and conditions outlined in the waiver statement below for each participant, including children.

I approve my child’s participation at all Rundown Sports LLC. I expressly represent Rundown Sports LLC. that my child is in good health and physically capable of participating in all activities sponsored and associated with Rundown Sports LLC. I authorize Rundown Sports LLC or its representative to request and obtain emergency medical treatment for myself or my child as the circumstances may require and in connection with this authorization I hereby waive and release the right to authorize and give consent for the delivery of medical care/treatment, of whatsoever kind and nature to my child. I understand that Rundown Sports LLC, its staff members, associates, workers and anyone associated with Rundown Sports LLC is harmless and release them from all liability for injury as a result of my child’s participation in any activity sponsored by Rundown Sports LLC. This release of liability by me is based upon the recognition that sport activity of any kind or nature clearly involves the risk of injury or hazards to the participants and spectators and I acknowledge that my child and I assume such risks when we participate in activities sponsored by Rundown Sports LLC. It is understood that once a player/parent signs this agreement and makes payment there will be no refund for any reason. By signing this agreement, the parents and players agree to abide by all the above and agree to give Rundown Sports LLC the right to put their child’s profile information on the internet. Also, the right to use photographs or videos of my child for promotional purposes in any type of media, including our website and I understand that I or my child will not be paid or compensated for providing this authorization.
I acknowledge that sending my son to practice, camps, or events with Rundown Sports LLC. is a volunteering and personal decision. I assume such risks when we participate in activities sponsored by Rundown Sports LLC.
